      I like it.  Could failed notches subtract from initiative also (you're
  too busy freaking out to do anything)?

I wouldn't use that because:  A it creates a new type of roll in ua.  I like
that all the rolls are 1d100 and how well you roll determines damage, etc; B
it requires me to do math.  I know that sounds weak, but it adds a level of
complication that borders on silly.  (i.e. dave fails a violence check mid
combat, his initiative modifier changes for the duration).  C) I really like
the result of the roll equals initiative, or the half result, neither of
which require either of those two problems.
