Well, there are at least one or two magic fixes, aside from the ones you
mentioned.
For one, Avatars of the Mechant or the Gambler could give you an opportunity
to shift it off to some other poor soul. Then again, could you live with
yourself?
More importantly, what if the notches you pass off come back to haunt you?
Some dissatisfied customer grabs you and tries ritualistic trepanation to
siphon the crazy back into you.
Of course, it is not outside the realms of plausibility to have a mystically
aware psychyatrist.
Without spoilers, we have seen one mystic psychologist in training, who had
a bent towards altruism.
(Then again, her obsession will probably take her into a different career.)
Perhaps there are rumours of a good shrink in the Underground who will heal
your character's mind and won't call the orderlies when you tell him about
the magic pixies who stole your wallet.
You could play it as a sort of modern take on the mystic hermit. A master
who hides in the city instead of in a shack on top of a mountain?
Winning his respect, and finding him in the first place, could be a good
basis for an adventure.
