Some sort of terrible, wayward agent for a defunct
Room of Renunciation. The Agent is a mystical surgeon
that operates on people correcting them and them
shelves them to show off to clients.

Or even better. The tumour becomes animated because
the writer had cancer of the soul too. The cancerous
soul and tissue have fused together into some sort of
parasitic, vampiric, fluid-dripping abortion.

I wrote up an archetype for this called the Grotesque
Libertine. It's somewhere in the list archives and in
the UA website.

Start with the small ones at "witness level" start on
the nastier stuff as the game goes on.
Does the writer's cancer go into remission? Or does it
start over from scratch. Doeshe start budding these
hideous tumours regularly? Do they mobilse and attack
people in dim porn theatres and under-street bars?

It really depends on how the character perceives the
situation. To some it's violence, some it's Unnatural,
or Helplessness. Level five or six.

Running Man is the Pilgrim and Gambler or Judge fills
the Advocate role.

Run it in your home town, or nearby city.
Good luck.
Cheers,
Chris.
