That would be great. Such a questionaire is exactly what I'm looking 
for. It will help in making the characters feel more real to me and the 
players and I can also work the magic thing into it.

Some questions that come immediately to mind:

When was the last/first time you were really drunk?
Did you ever watch porn?
Who explains the birds and bees to the kids - you or your partner?
What was your favorite subject in school?
What party do you vote for?
Which religious belief - if any-  do you have?
What would you risk your life for? Your kid, your wallet, the kick?
Do you have tattoos, piercings or similar stuff?

any more ideas?
