Ken Hite did a whole Suppressed Transmission column on the subject, but
I'd be tempted to work it into UA in a couple of ways:

    1) The people who drill holes in their own heads are, as common sense
would indicate, deluded whack-jobs. Yeah, there are a couple of medically
sound reasons to put a hole in your skull, but depression isn't one of them.

    2) It's a misunderstanding by mundanes of an epideromantic spell to open
the Third Eye. Does it really increase brain-blood volume and all that?
Hell, no. But if you're an epideromancer, you know that it's actually part
of a spell that lets you see astral parasites or get a glimpse of the
statosphere or something. The guy who invented the spell would probably get
a good laugh out of all the idiots who believed the brain-blood volume
story, if his enemies hadn't found *another* use for that hole in his skull.

    3) For most people, the act of drilling a hole in your head does
nothing, but with the right ritual, you can force possessing demons to leave
through the hole.

    4) It's a supernatural con job. The process is actually used by
unscrupulous adepts to *put something in* your head.

    5) Any or all of the above.

A little adventure idea: A bizarre serial attacker is gassing people in
their homes and while they're paralyzed, he comes in and drills a hole in
their skulls. Why? See above.
