Those questions are not answered and (if I have my way and don't change my
opinion) they will not BE answered.  Presumably the GM can do what she
wants.  (One metagame idea someone [possibly me?] kicked around was this:
When your character ascends to the IC, YOU become the GM.  When the IC
fills up, it's time to start a new game.)
