I think he was talking about organ donation. Basically most of the countries
allow organ donation only if the donor is in a state of "brain death". If
Japan doesn't recognize this, then I assume it's not possible for a Japanese
person to donate an organ, and therefore for a Japanese person to receive
an organ in Japan. Hence they have to go abroad?

I hope I got everything right here, not completely sure about that :)
