Obviously, it's the picture of the carnal that's causing the controversy.
You're average store owner is not going to read every book that comes in cover
to cover.  What he will see is the demon that looks like a penis, and the last
thing he needs is a bunch of twelve year olds picking that up and giggling.
Also, if memory serves, there's a picture of a woman where you can see some
nipple.
