I think this is what you're looking for - Normative Appendix E (pg. 34)
has rates for health care facilities.

Hi All,
I am working on a project for a healthcare facility. The spaces are
mostly exam and dental spaces, as well as waiting areas. The guidelines
for LEED IAQ P1 state that for any spaces not covered by ASHRAE
62.1-2007, the designer should choose a category that best corresponds
to the space. Have any of you had health care facilities certified? If
so, what ASHRAE 62.1 categories did you use for each space type?
