it shouldn't be anything physical, that wouldn't necessarily be
personal enough - perhaps the healer demands:

his shadow
his sleep
his public aclaim

maybe the healer just needs the cancer itself for whatever nefarious
reason

or with the removal of the cancer, the character's obsession is taken
too - because the cancer's obsession has been removed, there has to be
balance. The character's new problem is to find out how to replace it
without taking the cancer back too.
