As for insanity, I think the ability for players to manipulate their starting insanity notches, and to take paradigm skills goes a long way towards 'personalizing' stressful situations depending on the PC"s background. I had a long lived character in a CoC game, that started with a few lost points of insanity because I had defined him as being a CIA operative in vietnam. Violence didn't bother him much, and popping a bullet into a helpless prisoner's head didn't faze him. On the other hand his first encounter with the mythos freaked his normally rational shit out. He never quite could handle the unnatural.
In UA I would bump his hardened violence notches, and give him the Paradigm skill of Trained Government Killer (Violence/Unnatural).
