[UA] Cruel Storytelling

I was thinking the other day the cruelest things I could do to my players.  I just tend to think about it rarely do any of unless they deserve.  And I thought how could I get them to have a test against all 5 madness gauges...  The only one that I could come up with is an Oneiromancer's blast where the person dreams that he going up to hug his mom/friend/girlfriend and suddenly he seperates in two before he gets there.  He gets pushed back and his doppleganger hugs whoever and then slowly pulls out a blade and slowly kills his love one while he watch.   He is unable to close his eyes, be heard, seen, or affect the world in anyway.  He is forced to watch his doppleganger destroy his life.  This goes on for the player for what seems to be months.  I think I covered all 5 gauges there just a little iffy on the isolation.  Any other suggestions to get all 5 gauges in for one incident?
