AFAIK, the term was originally used by radio operators- obviously enough, "five by five" indicates the strongest, clearest possible signal.

There were 5 lights, or bars on the digital units that measured the clarity and signal strength, bmuch like on cell phones. if you have all bars lit up, you're 5x5
