How about this.

The unborn is possessed by the soul of a magician. The
mother doesn't know this, it's just her baby which she
loves unconditionally.

Unfortunately the Cruel Ones have realised the
magician's trick and are looking for him.

By careful dream manipulation the unborn magician has
sent messages to the sleeping brain of the mother.

She has been getting insights from Jesus (whom she
believes is the father of her child) and the angels
(she's very religious and deluded) that devils are
after her child (just to make this more interesting)
and she is a diagnosed paranoid schizophrenic.

The foetus trying to communicate with her is like a
deaf-mute trying to communicate with a blind person.

In the womb the foetus magician is trying to perform
it's minor rituals and to stack as many together to
perform the Rite of Harmonius Thingy.

At the last second the foetus-magician performs the
ritual for accessing a Room of Renunciation.

They enter the room and a pregnant, crazy woman has to
face a rather irate agent, while outside the Cruel
Ones are "scratching" at the door.

The Cruel Ones will kill the foetus by removing its
soul, which might even kill the mother - at least make
the situation worse for her than it is.

Perhaps the desperate cat-and-mouse has been mentally
beneficiant for her - she has had to face terrors
everyone else said was illusionary.

How does the agent cope with this hysterical, pregnant
schizophrenic and Cruel Ones?

Perhaps the agent was in the middle of doing her work
on an unwilling victim.

This would make a fantastic con adventure.
