Have you played Silent Hill 2?  The opening bits are awfully similar to
that.  Later, the protagonist meets an evil woman that closely
resembles his dead wife, which you might be able to use (perhaps the
duplicates in Bi3P are of people important to the PCs rather than the
PCs themselves).
