It's really just rock/paper/scissors in Japanese drag. ("Roshambo Meets
Roshamon?")
Total Attack = Rock
Total Defense = Paper
Cautious Attack = Scissors
Therefore, a total defense beats a total attack but is vulnerable to a
cautious attack. A total attack beats through a cautious attack but is too
obvious to penetrate a total defense. A cautious attack can find the
weakness in a total defense, but doesn't have the sheer power to stop a
total attack.
It helps to have the context from the UY comics to understand a total
attack. There are many fight scenes where the opponents stare at each
other from ten feet away, then they both holler and charge one another.
Generally one dies. (That's why, in the RPG, if you get total attack vs.
total attack, the winner does DOUBLE damage.)

If the reinstatement of capital punishment was clearly doing nothing to
reduce the number of murders in those states permitting it, it certainly
appeared to be cutting down on the number of confessions.
