I spend about 2 or 3 hours a week, usually while going somewhere on the train,
coming up with ideas and scribbling them down. I try to focus on images and
situations, high-quality things I don't trust myself to come up with on the fly.
Takes about another hour to work out some NPCs and a map or two. Then, because
my group are playing the reality cops narrative, I lay the week's 3 or 4 'cases'
on their table, fully prepared, and let them run with them in any order and in
any way they like. If anything crops up that I didn't expect, I run with it and
then write it up afterwards.

I try to reactive and avoid railroading; preparing multiple plotlines is my
concession to the PCs being masters of their own destiny. The interaction
between the plots, especially if they are connected and the PCs don't know it,
provides some very unexpected and unlikely results.
