It's happened before - check some of the old Darwin Awards.

The case I heard of involved a drunk man trying to get his keys out of a
toilet - he got his arm stuck, accidentally hit the flush lever and
becasue teh water couldn't leave the bowl (his arm wasblocking the pipe)
he drowned.


For use in UA - 

This would be another one of those 'strange loooking but very mundane'
occurances you can throw in as a red herring.  PC's head over to talk to
someone, find him drowned in a toilet...



And for the record I'd never heard of Mr. Savoie until this post.
