For a teen who went to a doctor against his will, I'd say
each visit would warrant a Helplessness check.
For people who hate needles but recognize the necessity of
them, that might be a Violence check. However, I would
still view that as a Helplessness check. After all, you're
still being forced against your will to take that insulin
shot. It doesn't matter if it's because your mother forces
you or the fear of dying forces you.
By that reason, I guess that Diabetes victims should have
at least one hardened notch in Helplessness.
Of course, if people don't fear needles, then no check
should be necessary. I don't have the comparative chart in
front of me, but I imagine that needles would be only a
level 1. Your parents dragging you to an evil man in a
white coat and a needle as long as your arm would be
higher.
A sadistic doctor who jams the needle into your face and
purposely scrapes the sharp point across your cheekbone,
ripping your skin as he twists the needle, would definitely
warrant at least a rank-4 Violence check (that's the rank
for random violence, right?). If that is your perception
of the benevolent old doctor, then the Violence check would
probably override the Helplessness check.
